The post holder will provide specialist nursing advice and support to patients with Heart Failure attending Cardiac Rehabilitation, and facilitate high quality, individualised care to patients/ clients and relatives/carers, in collaboration with the multi-disciplinary cardiac rehabilitation / heart failure team in line with local and national targets. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
To be flexible in the approach to the clinical area and to provide cover for the Cardiac Rehabilitation service acting as professional role model providing direct and indirect clinical care, teaching and research.
To develop the Cardiac Rehabilitation Nurse (Heart Failure) role in accordance with local and national policy and ensure the needs of patients and their families are central to service improvement initiatives.
· To deliver a home-based cardiac rehabilitation programme to patients and carers via the Reach Heart Failure Manual Programme.
· Communicate and explain the programme to patients and their carers in an individualised, compassionate, and caring manner.
· Motivate and encourage patients though their cardiac rehabilitation journey.
· To work as a specialist practitioner to support the effective delivery of a nursing service within the cardiac prevention and rehabilitation service.
