OBJECTIVE
The purpose of this discussion is to consider how individual factors can impact the health assessment. These factors include but are not limited to age, gender, mental health, literacy, disability, and culture. Students will discuss the impact of individual factors on the assessment and then collaborate on strategies to adapt the environment and assessment techniques to best facilitate the provider-patient relationship and data collection.

In order to obtain full credit, you must interact on more than three days. INITIAL POST
Create a patient who has presented to the office for a health assessment as a new patient. Provide the demographics of the patient and background that includes factors that impact their care. Please then discussion how the age and these other individual factors have the potential to impact the provider-patient relationship and the health assessment (both subjective and objective findings).
These individual factors may include any factor that has the potential to impact the relationship or the assessment. Examples are age, gender, mental health, literacy, disability, culture, and language.
EXAMPLE:J is a 2-year-old female who is brought in by her grandfather for ear pain. She is new to the practice. J and her grandfather are from Burma and her grandfather speaks minimal English.
I would then go on the describe how J’s age, the language barrier, and her culture could impact the provider-patient relationship and the health assessment.
Your initial post should use at least three scholarly sources to support your description of how the relationship and/or assessment may be affected.

Classmate 1: Michael
GP is a 76-year-old male who has come into the office for a new patient visit with his daughter who drove him. He comes in for new onset abdominal discomfort after eating. He is of Italian descent and speaks moderate level English has been retired for several years from the local factory. Reports eating 2 heavy meals a day for many years, and has brushed off his symptoms for some time, because he states “ I thought it was normal and didn’t want to come but my daughter made me” He also reports smoking cigars daily with his friends at the social club and having a 1-2 glasses of wine daily. He has a history of hypertension and hyperlipidemia for which he is compliant with his medication.
In this case we must consider the patients age, ethnicity and socioeconomic background. Understanding the population, you serve is vital in order to practice effectively in the community, each population and community have different social economic backgrounds with a variety of health determinants (Beehler et al., 2019). Many do not seek treatment due to the thoughts of perceived need to seek care, and the thought of taking care of it themselves which is highly common amongst the immigrant population in this country. This can impact the patient- provider relationship because they do not come in to seek care and when they do it often turns into a more serious matter such as more symptom development or further conditions arise (Saint Arnault et al., 2018). This patient’s age also plays a role in the relationship due to a delay in diagnosis, and the start treatment in the elderly, which may be due to trust of their socioeconomic status such as their finances, and transportation which is a major factor with elderly patients (Srivastava et al., 2018). As an elderly male its important for the clinician to understand that the healthcare of this patient and many other elderly patients requires a systematic review of not only a physical exam but a review of their socioeconomic status and resources available to them (Ward & Reuben, 2022).
This patients culture also effects the relationship of the provider and patient because culturally he eats a lot of heavier foods daily including tomatoes and coffee and wine which can be causing his symptoms as well as smoking which are risk factors. Spirituality and religion also play a role in the patients healthcare, they perceive that they are still alive and healthy and not yet deceased (Jantapo & Kusoom, 2021).
Building trust between the provider and patient is key for the patient to follow recommendations made by the provider. As well as some of these recommendations might not be followed but can potentially be decreased in frequency to assist in symptom management. We as providers might not be able to change the patients ways completely but we can provide education and hope they tweak their lifestyles in some way towards a better and healthier way of living, as we must understand we cannot change culture or socioeconomic status but we can educate and hope for change in some way.
Classmate 2: Amanda
D is a 78-year-old male who presents to his primary care provider with his wife for his annual wellness examination. He was born in Colombia and moved to the United States about 2 years ago with his wife so that they could live closer to their children and grandchildren. He and his wife do not speak much English and have a primary language of Spanish. D has a past medical history of osteoarthritis, hypertension, and was recently diagnosed with Alzheimer’s disease. The provider has been caring for D since he moved to the United States, so she is familiar with him and his wife.
Recognizing that the patient may have cultural beliefs that influences his perspective on health and illness, his communication techniques, and his views on recommended treatments is essential (Betancourt et al., 2021). It is also important that the provider focuses on building trust with the patient by demonstrating therapeutic listening, exploring the views of the patient, communicating effectively, being honest and respectful, and developing a working partnership with the patient (Betancourt et al., 2021). According to Betancourt et al. (2021) “The patient-based approach to cross-cultural care and communication entails four components: assessing core cross-cultural issues, exploring the meaning of the illness, determining the social context, and engaging in negotiation”. It is important to note that D’s primary language is Spanish. Bickley et al. (2021), states that even when communicating with patients who can speak some English, a medical interpreter should be utilized for the duration of the patient-provider interaction, as the patient’s full story is a key component to a detailed health assessment. Research suggests that language barriers may affect the quality of healthcare delivered to patients, with those who speak a different primary language often facing increased adverse events and seeking healthcare services more often (Shamshi et al., 2020). Therefore, it is important that the provider recognizes how impactful language barriers may be and attempts to diminish the possibility of adverse events or unsatisfactory patient experiences through appropriate communication methods (Shamshi et al., 2020). Lack of appropriate management of language barriers amongst healthcare providers can directly affect patient satisfaction, safety, and the interaction between the patient and provider (Shamshi et al., 2020). In this case, a medical interpreter should be used, and the provider should follow appropriate guidelines to ensure the interaction is successful, taking time to perform introductions amongst all communicators, sharing the goals of the interview, being transparent about the purpose of the interpreter, keeping the focus on the patient throughout the course of the assessment, using simple language when possible, and showing appreciation for the interpreter and patient at the end of the interview (Bickley et al., 2021).
D’s cognitive status is another factor that should be taken into consideration when determining an appropriate and effective health assessment approach. Because he has a known diagnosis of Alzheimer’s disease, it may be difficult for him to accurately recollect and describe his health history or share his concerns (Bickley et al., 2021). It is also possible that D may not be able to offer his own history, in which case it might be most appropriate to obtain details from his wife (Bickley et al., 2021). Family caregivers are an excellent source of information for the provider caring for a patient with Alzheimer’s disease and communicating with family caregivers stimulates positive communication and high-quality patient-centered care (Griffin et al., 2019). Regardless of the severity of impairment related to the patient’s Alzheimer’s disease, promoting family involvement during healthcare appointments, even early in the disease, can encourage patient support and ensure accuracy of information (Griffin et al., 2019). However, it is important to note that disclosing patient health information to family members who are not the patient’s healthcare proxy during the interview is not appropriate without the patient’s consent
