Directions:Foreach clinical week, write personal learning objectives. Analyze your ability to
meet each objective as partially met, fully met, or not met. Provide a summary about why you
rated yourself as you chose to and provide examples, which support your self-assessment.
Revise your learning objectives on a weekly basis. It is your responsibility to discuss your
objectives with your preceptor and follow his/her recommendations.
Please type these, use as much space as you need to reflect on your progress, print and keep
attached to your timesheet so that your clinical instructor may review as he/she requests. This
can either be handed in or uploaded into the CANVAS class discussion section.
