Assignment Description

Please read the following scenario and Develop three (3) SMART objectives to help your organization meet best practices and “close the gap”.
Organization Profile
The primary care adult health clinic consists of four physicians, three NPs, six staff nurses, one office manager, and several support staff. This clinic is one of 30 primary care and specialty clinics that make up a large macrosystem health service organization.
Background
Headache is a common neurologic disorder producing considerable disability (Rizzoli & Mullally, 2018). Half to three quarters of adults aged 18 – 65 years in the world have had headache in the last year and, of these, 10% experience migraines. The top three causes of consultation for headache, in both primary and specialist care, are migraine, tension-type headache and the combination of these (WHO, 2011). Although evidence demonstrates the effectiveness of a wide variety of therapeutic strategies for the treatment of chronic headache (Kennis et al., 2013), considerable barriers exist that frequently prevent optimal management. Lack of adequate patient education resources are among the factors that can impede effective promotion of headache self-management (Bromberg et al., 2011).
Problem Statement
Many clinicians lack the necessary expertise to accurately diagnose headache type and are unacquainted with the wide range of available therapies (Smith, 2010). Patients lack headache self-management skills and are more likely to overuse acute care resources, thus creating an enormous economic burden for patients, employers, healthcare systems, and society (Stokes, 2011).
