Case Study Assignment #1 Scenario Options – Due 3/2:
Please select one of the below scenario options and prepare a 2-3 page (500-750 words) APA formatted written response incorporating the Six Sigma and Lean tools explored in our course.
Scenario 1: Patient Experience in a Federally Qualified Health Center (FQHC) outpatient clinic: You are the Chief Operating Office of a federally qualified health center serving an underserved population of individuals and families in a rural MUA (medically underserved area). Resources have been strained during the COVID-19 pandemic leading to the closure of 1 of your 2 sites. With twice the patient volume funneling through 1 outpatient location, your internal patient experience/satisfaction scores have plummeted. Patients are dissatisfied with the timeliness, quality, and accessibility of their care at higher levels.
Scenario 2: Inpatient Critical Results Timeliness: You are a Director of Quality working in a large 480-bed general acute care medical center (hospital) facility. As a hospital facility, you are required to meet Joint Commission and CMS requirements for timely reporting of critical laboratory results (see NPSG 02.03.01). Your facility currently is not meeting the facility timeliness requirements (lab reporting the critical result to physicians within 60 minutes) potentially endangering patients who need treatment or medication changes in response to out of range lab values.
Scenario 3: Fall Prevention in a Skilled Nursing Facility: You are the nursing home administrator in a 140-bed skilled nursing facility. Over the last six months, attention has been diverted to COVID prevention and creating safe socially distanced activities and quality therapy for residents. Your fall rate has doubled over the last year with patients experiencing higher rates of falls and falls with injury. For this scenario, you may want to consider recommending an evidence based tool or approach such as this one from AHRQ.

What data/numbers will you collect? How will you measure the Voice of the Customer (VOC) and capture a baseline of the current situation?
What analysis tools or approaches would you recommend the team use to review the data and create an action plan?
