One of the goals in population health is improving the health of vulnerable and marginalized populations. These populations can be identified when looking at the determinants of health which are the physical, biological, social, cultural, and behavioral factors that influence health. APRNs serve as advocates for vulnerable populations. Two of the most effective ways to advocate for patients is through community collaboration/ partnerships and by creating health care policies. Through community collaborations, APRNs can help determine the health of the community through community health assessments.
Consider a community known to you. You wish to take steps to build a partnership within the community.
1. What data and information should you analyze and consider before you contact community leaders/members/organizations? As you answer this question, consider the following questions: What has been the history of this community during and following previous change and engagement efforts? How are the community and its various groups likely to perceive the APRN, and what is the potential for a successful engagement of community members?
2. Which area leaders/members/organizations would you contact initially to introduce the idea of a partnership?
