This report is to be submitted at the end of the Internship. The student Intern should discuss this assignment with their Field Supervisor and obtain his/her approval relative to its fulfillment. Since patient/client confidentiality is of utmost importance, do not identify the name of the client in your report. Select one patient/client with whom you will be working during your Internship. Keep notes of the patient’s/client’s progress during your intervention and submit a written Patient/Client Evaluation report to the University Supervisor at the end of your Internship using the following report format:
Patient’s/Client’s Longitudinal History (Milestones)
Patient’s/Client’s Current Medical, Psychological, Educational Background
Patient’s/Client’s Needs Assessment (explain assessment procedure used and include the following, if applicable)Task Analysis (observation)

Anecdotal Record of Client’s ProgramPhysical and/or Motor Objectives

Reinforcement Techniques
Summary of Patient’s/Client’s Progress During Period of Intervention
