Part 2: creating portfolio
You are in the early stages of creating an ePortfolio for this class. Although there will be specific requirements (to be revealed at a later date) that you will need to include in your ePortfolio for this course, you should be thinking beyond the course requirements to create a living, breathing portfolio that will continue to serve you as you progress in your capstone course, your education, and your career. This forum provides an opportunity to converse with your peers about portfolios, share ideas, and begin planning your approach to your own portfolio.

1. You have read about portfolios in this lesson. Please explain what you would like to accomplish by creating your own portfolio (i.e., purpose). Describe artifacts you think will be important to include in order to achieve this purpose.

A education brochure needs to be created based on attached health promotion plan.
This week you will use your Health Promotion Plan (Week 5) to create a patient education brochure. The information in your brochure should target a specific population group for your Healthy People 2030 topic and be organized, accurate, and informative as well as visually appealing using appropriate layout, design, and graphics.
You will create a 6-panel brochure that will inform and educate your population of interest using the following criteria:
Create an APA-formatted title page that will come before your brochure. In addition to the APA-required information on this title page, include your target audience and feedback from a member of your target audience. (Note: You will first create your brochure, share it to garner feedback, and then document this feedback on your submission.)
Review the 3 objectives you listed in your Health Promotion Plan. Utilizing these objectives and the content outlined in your Health Promotion Plan, develop content for your 6-panel brochure.
Use text, headings, and graphics to convey the content to your target audience for them to achieve the stated objectives.
Keep in mind the behavioral theory/model in the design and content of your brochure. Provide a reference for this on your reference page.
When writing your content, use simple and straight-forward language appropriate for your target audience. Proper spelling, grammar, and mechanics are required.
Include references and in-text citations for your sources. You need a minimum of three scholarly or professional sources. Include APA-formatted references on the back panel of your brochure.
Please note you will need to share your brochure with a member of your target audience and document their feedback on the title page. For example, “John Doe reviewed the brochure and indicated he had difficulty understanding some of the terms. The terms were modified.” You may not document feedback from a healthcare provider or from a family member.

In healthcare, the terms “balanced scorecard” and a “dashboard” are used to describe performance measurement tools that are broad in scope but summarized in a few key indicators. In this assignment, you will create a specific dashboard for an operating Week of a healthcare organization.
Tasks:
Select and describe a specific division of a larger healthcare organization, such as an emergency department, a surgical service, or a physician practice.
Locate at least three recent (within the past three years) journal articles from professional, peer-reviewed journals that discuss dashboard, balanced scorecard, and performance measurement in your chosen healthcare division.
Write a review of each article and include complete citations.
Based on the information learned, create and justify the need for four categories of measurement, such as customer satisfaction or financial performance. For each of these four categories, create three specific performance measures. For each measure, describe how it is calculated and/or where the data can be found.
Explain how the performance measures may change if the analysis is for the organization as a whole rather than a division of the organization.

Reflect on the conversation surrounding the need to consider complex systems and how you approach typical business processes in your organization. According to the second video, typical tools such as KPIs and other metrics may not adequately capture the needed practices to address complex issues in our organizations. Discuss what issues you were thinking about when watching that video and reading the system practice workbook. What is preventing you from managing these issues with a complex systems approach in mind?

Summarize ONE of the videos and include a short description about what the video is focused on. Give details about the human microbiome such as organisms that live there and how they affect us.

Assignment:
In our program, we are thankful to have students from a variety of disciplines – some are direct healthcare practitioners, some are corporate healthcare leaders, some are in other human services functions such as social services and public education. Still others come from many other backgrounds. This diversity is one of the things that makes the program so dynamic. The most important part of our diversity is that in one way or another, each of you comes from a background of making the world a better place for people.
One of the things we discuss during this term is the importance of recognizing, as a leader, how you substantively contribute to a larger mission through your actions – and how a system of actions makes positive impacts on the world. Regardless of your positional title (we have CEOs and front leaders alike in our program), as leaders, each of you are obligated to make your organization, community, and the world a better place through your actions. The purpose of the final exercise is to think about your team and your role in the world and how you can make significant progress on an issue about which you care greatly.
The United Nations describes its Sustainable Development Goals (or SDGs) in the following way:
“an urgent call for action by all countries – developed and developing – in a global partnership. They recognize that ending poverty and other deprivations must go hand-in-hand with strategies that improve health and education, reduce inequality, and spur economic growth – all while tackling climate change and working to preserve our oceans and forests.”
The SDGs provide a list of global goals that can apply to many of the fields in which each of you work. More importantly, we should think about how actions in our own organizations substantively contribute to the larger goals described in the SDGs – each of us have to contribute in our own, even sometimes small, way to make them successful. For example, none of us can solve global poverty individually, but working to solve poverty in our own community helps towards that global goal. For this project, you should choose one of the Sustainable Development Goals and develop a radical plan to make progress on this issue from where you sit in your specific organization, whatever that organization may be. You should research this goal on the UN SDG website (https://sdgs.un.org/goals) and choose one issue in one of the goals to improve. You should consider, as a component of your thought process, the Systems Practice Workbook from Week 1 (available on Moodle).
Please note that you are invited, and expected, to dream and think big, but you must have a clear and focused target audience and a very specific plan. Please note that I’m looking for direct impacts and, as such, proposing informational campaigns alone is not sufficient as a radical idea, nor are descriiptions of existing programs. Finally, I’m asking you to think outside the box and do something different from what’s need done before. Therefore, proposing COVID-19-specific interventions like vaccinations clinics or informational campaigns are not sufficient for this exercise (that doesn’t mean you can’t address other follow-along issues like provider burnout or the now known lack of quality public health capacity in many localities).
The final paper is a 2000-word report which will present an idea for a project your team at your work can undertake to make progress on one of the UN’s Sustainable Development Goals. Ideally, you should address how this can be implemented in the fastest and most effective way possible. This report should have your organization’s leadership as the audience in mind but requires a very limited introduction to the organization as needed for non-local audiences. Your report should include the following components and structure (the word limits for each line are just a guideline – you may deviate from this as necessary, but this gives you a general understanding of the detail with which you should address each topic):
• Section 1 – Introduction: Brief introduction to your organization and/or team. (100 words)
• Section 2 – Sustainable Development Goal: A brief descriiption of the SDG that your report will consider and your radical idea for accomplishing that goal. (300 words)
• Section 3 – The Problem and the Solutions – A brief discussion of the problem that you’re trying to solve and a brief descriiption of what your proposed solutions will be (350 words)
• Section 3 – Logic Model: A refined logic model of the existing policy or plan which sketches out the key assumptions related to its inputs, outputs, outcomes and impact. See https://www.acf.hhs.gov/sites/default/files/documents/prep-logic-model-ts_0.pdf for a discussion of logic models. Please consider a graphical representation rather than a narrative approach for this section. And remember to start from the outcomes in mind and work backwards! (100 words)

• Section 4 – Stakeholders: An approach to tactics building the team you need to accomplish this goal and helping to secure buy-in from your team and your chain of command (everyone has a boss!), which also addresses potential opposition to your idea. Your recommendations should include ideas for mobilizing key stakeholders to support the proposed policy or plan and to become agents of change. While a narrative in this section is critical, a graphical representation summarizing this information can be helpful. (350 words)

SDG #2 End hunger, achieve food security and improved nutrition and promote sustainable agriculture.
Working with local farms in Lampasas, Texas, to develop a prototype crop that can be an alternative food source that is drought resistant, doesn’t deplete nutrients from the soil, and has minimal requirements for machinery. In certain areas, the yield could be used as a cash crop. As a community, we can help end hunger locally to make a more significant impact globally.
