#1 Wrong Operation Doctor (ethics and integrity) Hospitals find it hard to protect patients from wrong-site surgery. Last year a jury returned a $20 million negligence verdict against Arkansas Children’s Hospital for surgery performed on the wrong side of the brain of a 15-year-old boy who was left psychotic and severely brain damaged. Testimony showed that the error was not disclosed to his parents for more than a year. The hospital issued a statement saying it deeply regretted the error and had “redoubled our efforts to prevent” a recurrence. So, what happened? “Health care has far too little accountability for results … . All the pressures are on the side of production; that’s how you get paid,” said Peter Pronovost, a prominent safety expert and medical director of the Johns Hopkins Center for Innovation in Quality Patient Care. He added that increased pressure to quickly turn over operating rooms has trumped patient safety, increasing the chance of error. Kenneth W. Kizer, who coined the term “never event” nearly a decade ago when he headed the National Quality Forum, a leading patient safety organization, said he believes reducing the number of errors will require tougher reporting rules and increased transparency.

Upon completion of this assignment, you will have used the Balanced Scorecard tool to define objectives, measures, targets, and initiatives for the health care organization you selected and described in the Week 1 assignment.
Review the 4 perspectives of a Balanced Scorecard described in this week’s readings and visually depicted in The Balanced Scorecard by Robert Kaplan.
Construct a Balanced Scorecard, using the Balanced Scorecard Template, for the health care organization you identified in Week 1. For each of the 4 perspectives, define at least 3 objectives, measures, targets, and initiatives. Ensure that the objectives are clearly aligned to the organization’s mission and vision described in the Week 1 assignment.
Write a 400- to 600-word executive summary in which you reference and include the Balanced Scorecard table you created. Describe the following in your summary:
How the objectives are aligned with the mission and vision
Why the measures and targets shown were selected
How the initiatives will help achieve the objectives

Identify examples of active and latent errors. Provide examples from your clinical experience, if possible. How can such errors be avoided to support better patient care?

Please submit Two files:
One should be the Microsoft Word file that includes both the essay responses and the tables from the Excel file (embedded or pasted) in the report or as an appendix. Please note, all of the information must be in this report. I will only use the Excel file to verify the analysis and will not use it to grade your report.
The second file must be the Excel file (same version as posted in Canvas) showing your calculations for the reliability and validity estimates and the actual SEM calculations. If you use regression to calculate the reliability and validity results, then add a tab in the Excel file and place those output files in a separate tab (one tab is fine for all of them).
