There are many clashing views in Food and Nutrition. One of them is the issue of high fructose corn syrup (HCFS). Joseph Mercola strongly considers HFCS more deadly than sugar due to the ways that the body converts fructose to fat. He accuses the Corn Refiners Association of trying to convince us that their HFCS is equal to table sugar. The Corn Refiners Association claims that HFCS has no adverse health effect and it is the same as sucrose and honey. They also emphasize the benefits that HFCS provides to food.
Important Dates: 1970s High Fructose corn syrup use in the United States began. 1980s HFCS is used in all non-diet soft drinks in the United States. Please review and research the two thoughts and include your opinion in the discussion post. Things to think about are: effects to health, links to diabetes, fructose from fruit, is there a link between the introduction of HFCS and the rise of obesity in the past 30 years? Does HFCS affect the feeling of fullness? You do not need to respond to each of these just include your thoughts!
Please view a clip from “Saturday Night Live” mocking the clashing views. It is located at “Saturday Night Live Corn Syrup Commercial”. Be sure to view the entire clip and include your thoughts in the post!

Subject: Health & Medical You will perform a history of an abdominal problem – irritable bowel syndrome (IBS). Perform an assessment of the gastrointestinal system. You will document your subjective and objective findings, identify actual or potential risks, and submit this in a Word document to the drop box provided.
Remember to be objective when you document; do not make judgments. For example, if the person has a palpably enlarged liver, do not write “the liver is enlarged probably because they drink too much.” Avoid stating that something is normal but instead state WHY you think it is normal. For example, if you think that the abdomen looks “normal” – which is subjective – then document that the “abdomen is flat, skin color consistent with rest of body, no lesions, scars, bulges, or pulsations noted.”
In acquiring subjective data of an abdominal/gastrointestinal examination, remember to ask about:
normal bowel patterns and if there are any changes in the bowel movement. normal bowel movement varies among individuals and knowing what’s normal will help the nurse identify if there is a problem
normal diet. is the patient vegetarian, vegan, etc. what does she normally eat? did he change it?
other symptoms such as nausea, vomiting, flatus
any unexplained changes in weight. any significant weight loss may delay wound healing if the patient is scheduled for surgery
In acquiring objective data, follow the steps below:
Inspection. Inspect the shape of the abdomen. Is it flat, round or globular. Look for any lesions, scarring and ask the patient what is normal for him to help differentiate what is not normal.
Auscultation. It is important to do auscultation before any manual examination is done to the patient. This is as to avoid any movement which may alter the result of the auscultation. Listen for bowel sounds and determine if it’s normal.
Percussion. Percuss and determine if the abdomen is tympanitic. Also determine if the liver size is normal by percussing to the liver edge and determining the size.
Palpation. Look for rigidity or guarding. Also look for masses, if warranted.
Save your assignment as a Microsoft Word document.

RESPOND TO THIS
The four Lead up principles that I read in the book were all quite interesting to read and learn more about. Lead-up number 4, which is “Do More Than Manage – Lead!” is something I constantly see in the show Grey’s Anatomy. Throughout the seasons we see five interns growing and learning how to lead within the show. More specifically, we see Richard Webber go from being a great chief, being a bad chief, and losing his position to then regaining his chief status. I recently rewatched an episode of a shooting that happens when Dr. Webber was not the chief at that point. He had lost his chief role from alcoholism and needed to go to rehab. While the hospital and its staff have been recovering from the incident, Dr. Webber stepped up again and starts to manage his hospital in a very different way as something tragic has happened, and now has to work around the PTSD from that event. We also see Dr. Webber start to lead and let the residents start stepping up in an attending role as one of the residents had to do surgery by herself with a gun to her head trying to save her attendings life from a bullet wound. This is a point from Leap-Up 4 of “Leaders Push Boundaries” as Dr. Webber wants them to find a better way for his staff to work in their new environment settings with PTSD. We also see Dr. Webber making improvements by adding cameras and security systems so that the incident that happens does not happen again and keeps his hospital staff/patients/visitors safe. Also around that same time period, Dr. Webber starts to invest power in others, which is another point from Lead-up 4, where he sets out a competition for the attendings and gives the prize of 1 million dollars to the attending that has newer ideas to better their department and to create newer medicine and show that they can overcome anything and come back even stronger than before.
Make sure to use some points from this I enjoyed reading the second principle about how motives matter and lifting the load. This particular part of the book reminds me of my father, Kevin. My dad owns a business and usually works Monday through Friday. However, occasionally, I will call him on Saturdays to find out he is at work. I asked him why he works on Saturdays when that is his off day once, and his response was, “If I expect my employees to work hard, love their job, and respect me, I also have to work hard, love my job, and respect that they need help; even on the days I want to be at home.” That is something that has motivated me to work hard and go the extra mile in the things that I do. Another reason this reminded me of my father is growing up, I felt like I constantly had to please my dad. Either through academics, sports, being active in church, etc. I remember being terrified to tell my dad that I was moving halfway across the country for a job that I loved because he would think it was dumb. When I told him, he responded, ” if that is what you love, you need to do it.” Afterward, I told him how nervous I was to talk to him about moving, and his response was, “if your motivation is to please me, you will never be able to enjoy the things you do.” Don’t get me wrong, I still struggle with people pleasing and doing the right thing for the wrong reason, but with the help of my father, I can take on more responsibility to help others, with a motivation that is to be a better leader instead of to people please

    Hello Everyone-
Following is the case study for week three. This is a bit early; you may begin when you want to begin.
    Week 3: HEENT. 1
Use the following case to complete a focused SOAP note. Incorporate answers to the questions into your SOAP write up.
A mother brings in her 14-year-old son, Branch, to the Urgent Care where you are as a nurse practitioner. He has had a nosebleed. You learn that Branch and his mother live nearby the UC but they do not live near his school. He must take the train to the school and there also is a walk. Her first concern was that he had been attacked during the walk. Branch denied that and reminded them that he has no bruises. The nosebleed began during his sleep and continues this morning so that he could not go to school. The bleeding is bright and a small but steady stream from his right nostril. He has no history of nosebleeds, sinus or allergy problems and uses no nasal sprays. The mother asks how much the cost will be at the UC as she has no health insurance and just lost her Medicaid for them because she received a raise at her job. She says he needs to be seen, even if it is expensive, because they have been applying pressure by pinching it and the nosebleed won’t stop. You ask for additional information and learn that he has no significant medical history and no known allergies. He is on no medications. Branch does play sports at school, depending on the season. He does not recall any facial injuries at all. He says he just woke up with a nosebleed and it won’t stop. He tells you he is worried he will faint.
    Vital signs: BP 108/70 P 88 R 18 T 97.8 oral Pulse ox 99%
You recognize that simple pressure is not going to stop the nosebleed so you know that you will have to intervene.
Prior to any type of procedure, you have the mother sign an informed consent. What are the three major areas you must discuss when doing any type of procedure?
What are the three groups of nosebleeds and which is most common?
Name 4 indications for intervention by a provider for a nosebleed.
You place Branch on the exam table at approximately 45 degrees. You drape him appropriately. You have him blow his nose gently to remove clots. You then inspect the right side to familiarize yourself with his anatomy. You then inspect the left side using a nasal speculum. How should the nasal speculum be used and why is it important to use the nasal speculum a certain way?
You note that the bleeding is coming from an area on the septum. You know that the next step is to apply a vasoconstrictive solution to the nose. What are two ways you can deliver the vasoconstrictive solution?
You note that the area that is the source of the bleeding is about 3 mm in diameter. You make the decision to use a silver nitrate stick. How long should you apply pressure with the stick? Why it is it important not to use the silver nitrate for over that time frame?
After hemostasis is obtained, what are three types of treatment methods that can be used to protect the cauterization site?
If that had not stopped the bleeding and you had to make the decision to use a nasal sponge or nasal tampon, what should the sponge/tampon be coated in and how long should it be left in place?
After putting in the nasal sponge/tampon, approximately 2 ml of ________ or _______ should be dripped onto the tip to help the sponge expand.
After placing the nasal sponge/tampon, the patient should be closely monitored for 3-5 minutes. Why is that?
How long should the patient be kept in observation status?
If a sponge/tampon is used, is it necessary to use antibiotics?
If it is necessary to pack the nose, it may be advisable to give the patient a narcotic or sedative medication (unless a contraindication exists). Why?
Name 5 complications of the above procedures.
After the procedure, you tell Branch and his mother that he can take acetaminophen for any pain/discomfort. Why is it important not to have him take ibuprofen?
What is the leading cause of nosebleeds in adolescents?
What CPT code would you use for the above procedure? What is the definition of the code?
Replies to this email will be posted as a reply to the announcement, which will be seen by everyone in the course.
    To Prepare:
Review this week’s Learning Resources. Consider how to assess, diagnose, and treat patients with conditions of the head, eyes, ears, nose, and throat.
Review the case study provided by your Instructor. Based on the provided patient information, think about the health history you would need to collect from the patient.
Consider what physical exams and diagnostic tests would be appropriate in order to gather more information about the patient’s condition. Reflect on how the results would be used to make a diagnosis.
Identify three to five possible conditions that may be considered in a differential diagnosis for the patient.
Consider the patient’s diagnosis. Think about clinical guidelines that might support this diagnosis.
Develop a treatment plan for the patient that includes health promotion and patient education strategies for patients with HEENT conditions.
    The Assignment
Use the Focused SOAP Note Template to address the following:
Subjective: What details are provided regarding the patient’s personal and medical history?
Objective: What observations did you make during the physical assessment? Include pertinent positive and negative physical exam findings. Describe whether the patient presented with any morbidities or psychosocial issues.
Assessment: Explain your differential diagnoses, providing a minimum of three. List them from highest priority to lowest priority and include their CPT and ICD-10 codes for the diagnosis. What would your primary diagnosis be and why?
Plan: Explain your plan for diagnostics and primary diagnosis. What would your plan be for treatment and management? Include pharmacologic and non-pharmacologic treatments, alternative therapies, and follow-up parameters as well as a rationale for this treatment and management plan.
Reflection notes: Describe your “aha!” moments from analyzing this case.
