    Full-Spectrum Nursing Model With Blake
    Instructions
Using the client information provided, respond to the critical thinking questions. Each response should be original (in your own words) and a minimum of three sentences in length.
    Client Information
    Meet your client, Blake.
You have just received a report from the emergency department (ED) on a client named Blake. According to the ED report, Blake is being admitted due to chronic renal failure. He is married and an employed 58-year-old man, and he has a long-standing history of Type 2 diabetes mellitus (DM). During the past three days, he reports that he has developed swelling and decreased sensation in his legs and has difficulty walking, which he describes as “slight loss of mobility.”

    Critical Thinking Questions
List five questions that will help you assess and plan immediate and long-term care for Blake.
Based on the information provided and the questions listed, what are the priority problems?
Identify at least two resources you can use to find out more about the pathophysiology of renal failure. How do you know the sources are credible? As you are assessing Blake, who is your best source and why?
Write one collaborative problem statement for Blake. If you do not know the potential complications of chronic renal failure, look them up in a medical-surgical or pathophysiology resource. Explain why you would not use a nursing diagnosis to describe the problem.
Aside from his physical condition, what is at least one psychosocial concern Blake might have right now? In other words, what else might Blake want to have resolved that could–for him–be more important than his chronic renal failure?

You have probably seen one or more of the many inspirational posters about decisions. A visual such as a forked road or a street sign is typically pictured, along with a quote designed to inspire.
Decisions are often not so easily inspired. Perhaps you discovered this when choosing a specialty within the MSN program. This decision is a critical part of your plan for success, and you no doubt want to get it right. This is yet another area where your network can help, as well as other sources of information that can help you make an informed choice.
Note: Unless otherwise noted, initial postings to Discussions are due on or before Day 3, and response postings are due on or before Day 6. You are required to participate in the Discussion on at least three different days (a different day for main post and each response). It is important to adhere to the weekly time frame to allow others ample time to respond to your posting. In addition, you are expected to respond to questions directed toward your own initial posting in a timely manner.
    To Prepare:
Reflect on your decision to pursue a specialty within the MSN program, including your professional and academic goals as they relate to your program/specialization.
Post an explanation of your choice of a nursing specialty within the program. Describe any difficulties you had (or are having) in making your choice, and the factors that drove/are driving your decision. Identify at least one professional organization affiliated with your chosen specialty and provide details on becoming a member.
Respond to at least two of your colleagues’ posts by sharing your thoughts on their specialty, supporting their choice or offering suggestions if they have yet to choose.

    2 hours
Additional Time for Study, Research, and Reflection:
    4 hours
    Directions:
For this discussion, review the various healthcare settings in your reading (such as hospital, community health center, etc.) and select one. Once selected, research the internet to identify a facility that represents your selected healthcare delivery setting. Review their website and determine:
    • Location and size
    • Mission and values
    • Accreditation and awards
    • How long they have been serving their target population
    • Whether they are part of a system or a single institution
    • Are they for profit or not-for profit
    • The target populations they serve
    • The clinical and client services they provide
    • The type of staff who work in the setting
    • Contributions to the community
In a two to three paragraph discussion post, identify key aspects of the selected healthcare setting, how their mission and values compare with the services they provide and their target populations, the type of staff who work there, any special or unique aspects of the healthcare setting, and your impression of the healthcare setting after reviewing their website. You don’t need to identify the name of the healthcare setting just the type.

One of the tools that I need to implement to address SDOH is indices. Based on the analysis, the
Health Opportunity Index (HOI) is a univariate method that can be utilized more effectively to recognize
and comprehend the complex interactions of intricate social determinants of health (SDH) at the census
tract level that affect the ability to attain optimal health. The vulnerability index for each census tract is
found by adding up the number of flags in that area. The Robert Wood Johnson Foundation and the
Commonwealth Fund developed this tool. When calculating the HOI, people use a method called principal
component analysis, which is a kind of data reduction, to identify the influence SDH has on optimum health
in lower census areas. People highlight the potential effectiveness of using 13-input factors to derive a
reinforced metric of health (HOI) score to help recognize the most vulnerable members of society during
the current pandemic (Ataguba & Ataguba, 2020). This is done amid continuing health inequities and
the current COVID-19 pandemic. As indicated, the findings pinpointed census tracts associated with
populations at risk for unequal COVID-19-related health outcomes.
The HOI is one example of a tool that can be used to illustrate how the SDOH can improve the
health of a community as a whole. The Health Outcomes Index (HOI) is a way to find the local factors that
lead to bad health outcomes. To better target health improvement efforts, the HOI can be used to pinpoint
low-performing areas and eliminate health inequalities (Ataguba & Ataguba, 2020). This way, census
tracts with the poorest values for diverse health outcomes were easily identified using convergence analysis.
Based on our findings, the HOI composite score and the creation of additional scores may guide
mitigation/intervention practices, potentially allowing for more precise targeting of mitigation/intervention
tactics for susceptible populations during outbreaks like the current pandemic.

Our knowledge and understanding of the various forms of lymphocytes continue to expand. Our initial discussion this week will be on the various forms of B and T cells — each of us should take one class of lymphocytes and explain it’s role in the adaptive immune process. As you comment on other students’ postings be sure to add additional insight into the mechanisms of the subclass of cell and its interactions with both components of the innate and other aspects of the adaptive immune system.
Recently a number of individuals have claimed that adjuvants, such as thymersol in vaccines, lead to the development of a variety of disorders, such as autism. Although these initial claims have not been substantiated and have since been withdrawn from the scientific literature, the concern continues today. How would you counsel parents concerning vaccination? Discuss the issue of herd immunity and the risks in our modern society related to refusal to provide vaccinations
