As a nurse, you understand the challenges facing under-served populations. For this practicum assignment, you will identify a vulnerable, at-risk population in your community. You will examine the challenges currently facing members of this group and the resources available to assist them. You will gather information from a variety of sources and interview a professional who is involved in the provision of services to this group. A vulnerable or at-risk population can be defined as a population with a common identified risk factor or risk exposure presenting a threat to health. Examples of at-risk populations include, but are not limited to the following:
    Urban or rural individuals and families living at or below the poverty line
    Immigrants and non-English-speaking individuals
    Stigmatized or marginalized groups
    Undocumented workers
    Incarcerated individuals
    The homeless
    The mentally ill
    This five-part assignment is submitted in weeks 3, 5, 8, and 10. Please refer to the Practicum Assignment link on the navigation pane for an explanation of all steps. Please read it in its entirety before starting.

    You will work on this assignment over weeks 1-3. It is submitted at the end of week 3. In this assignment, you will:
    Identify your vulnerable, at-risk population of interest.
    Complete a needs assessment, analyzing data available through scholarly references that is specific to the identified population. At least 3 references should be used to complete the needs assessment
    Present your needs assessment data by filling out the attached worksheet below. Fill out the chart and answer the questions listed on the worksheet. Attach a reference page to the worksheet to list your references utilized in step 2.

    Pete is a 24 year-old African American man who was diagnosed with sickle cell disease when he was 3 years-old. He has a history of acute chest syndrome, necrosis of the
    hip, and has had recurrent chronic leg ulcers for 3 years. He had been receiving his treatment
    from a Pediatric hematology group and recently transferred to the adult hematology clinic. Since the transfer, he has been hospitalized monthly for sickle cell pain crisis, with back, leg and chest pain which does not respond to his home pain regimen of Dilaudid. Pete has monthly pain crises at home, despite following an oral pain regimen that seemed to be working at home. After being admitted to the hospital via the ER, Pete is usually hospitalized for 1-2 weeks on an IV pain regimen, which he heard about from his peers with sickle cell disease. This specific combination of medication that he asks for is known to cause a temporary “high” and Pete reports it is effective. He has asked that the palliative medicine physician be removed from his case because they consistently push back on prescribing this combination of medication. Pete has a good relationship with the palliative care social worker and consents to them continuing to follow. Pete has been offered home health care services, but he refuses. He is cared for at home by his mother, who also cares for his two younger siblings, ages 14 and 17. He has good support from his mother. Pete is Christian and considers himself very religious. During one social work interaction, Pete revealed an incident of sexual harassment by an authority figure during his adolescence but refused to discuss further when pushed.

    The SDOH framework suggests that health outcomes are contextualized within the wider social environment in which a person works, learns, and lives. It also incorporates a life course perspective, acknowledging that influences in early life can predispose an individual to poor health in adulthood and later life (Blane, 2006). The SDOH framework further acknowledges psychosocial processes, recognizing the relationship between external stressors (e.g., financial issues) and internal responses (e.g., biological responses such as raised cortisol; Brunner & Marmot, 2006). Since the original report on the SDOH in 1999, a growing body of research evidence supports the SDOH framework, which has been used to facilitate changes in policy and practice aimed at reducing inequality and improving health outcomes (Marmot et al., 2020). However, progress to address inequalities and improve the health of individuals and societies has been reported to be slow (Marmot et al., 2020).
