Student post: Hello, it seems like I’m responding to your post than anyone else. I always look forward to reading what you wrote, thank you. I noticed that you brought up communication as a topic. I have to totally agree. I remember doing my clinicals at one of the Banner facilities and noticed that communication was broken and could have harmed the patient. An ambulance came to pick up a patient, the nurse had no idea, no one knew who gave or received report. The paramedic just had an order to pick up the patient and that was it. The paramedic didn’t have a history just the name and DOB. SBAR was not used in any way and this could have harmed the patient. I do have to agree with you that communication is very essential in any medical field because a life is on the line.

Assessment 1 Instructions: Enhancing Quality and Safety
For this assessment, you will develop a 3-5 page paper that examines a safety quality issue pertaining to medication administration in a health care setting. You will analyze the issue and examine potential evidence-based and best-practice solutions from the literature as well as the role of nurses and other stakeholders in addressing the issue.Health care organizations and professionals strive to create safe environments for patients; however, due to the complexity of the health care system, maintaining safety can be a challenge. Since nurses comprise the largest group of health care professionals, a great deal of responsibility falls in the hands of practicing nurses. Quality improvement (QI) measures and safety improvement plans are effective interventions to reduce medical errors and sentinel events such as medication errors, falls, infections, and deaths. A 2000 Institute of Medicine (IOM) report indicated that almost one million people are harmed annually in the United States, (Kohn et al., 2000) and 210,000–440,000 die as a result of medical errors (Allen, 2013).The role of the baccalaureate nurse includes identifying and explaining specific patient risk factors, incorporating evidence-based solutions to improving patient safety and coordinating care. A solid foundation of knowledge and understanding of safety organizations such as Quality and Safety Education for Nurses (QSEN), the Institute of Medicine (IOM), and The Joint Commission and its National Patient Safety Goals (NPSGs) program is vital to practicing nurses with regard to providing and promoting safe and effective patient care. You are encouraged to complete the Identifying Safety Risks and Solutions activity. This activity offers an opportunity to review a case study and practice identifying safety risks and possible solutions. We have found that learners who complete course activities and review resources are more successful with first submissions. Completing course activities is also a way to demonstrate course engagement.Demonstration of ProficiencyBy successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:Competency 1: Analyze the elements of a successful quality improvement initiative.Explain evidence-based and best-practice solutions to improve patient safety focusing on medication administration and reducing costs.
Competency 2: Analyze factors that lead to patient safety risks.Explain factors leading to a specific patient-safety risk focusing on medication administration.
Competency 4: Explain the nurse’s role in coordinating care to enhance quality and reduce costs.Explain how nurses can help coordinate care to increase patient safety with medication administration and reduce costs.
Identify stakeholders with whom nurses would need to coordinate to drive quality and safety enhancements with medication administration.
Competency 5: Apply professional, scholarly, evidence-based strategies to communicate in a manner that supports safe and effective patient care.Organize content so ideas flow logically with smooth transitions; contains few errors in grammar or punctuation, word choice, and spelling.
Apply APA formatting to in-text citations and references exhibiting nearly flawless adherence to APA format.

To err is human: Building a safer health system. Washington, DC: National Academy Press.Professional ContextAs a baccalaureate-prepared nurse, you will be responsible for implementing quality improvement (QI) and patient safety measures in health care settings. Effective quality improvement measures result in systemic and organizational changes, ultimately leading to the development of a patient safety culture.ScenarioConsider a previous experience or hypothetical situation pertaining to medication errors, and consider how the error could have been prevented or alleviated with the use of evidence-based guidelines.Choose a specific condition of interest surrounding a medication administration safety risk and incorporate evidence-based strategies to support communication and ensure safe and effective care. For this assessment:Analyze a current issue or experience in clinical practice surrounding a medication administration safety risk and identify a quality improvement (QI) initiative in the health care setting.
InstructionsThe purpose of this assessment is to better understand the role of the baccalaureate-prepared nurse in enhancing quality improvement (QI) measures that address a medication administration safety risk. This will be within the specific context of patient safety risks at a health care setting of your choice. You will do this by exploring the professional guidelines and best practices for improving and maintaining patient safety in health care settings from organizations such as QSEN and the IOM. Looking through the lens of these professional best practices to examine the current policies and procedures currently in place at your chosen organization and the impact on safety measures for patients surrounding medication administration, you will consider the role of the nurse in driving quality and safety improvements. You will identify stakeholders in QI improvement and safety measures as well as consider evidence-based strategies to enhance quality of care and promote medication administration safety in the context of your chosen health care setting.Be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so that you know what is needed for a distinguished score.Explain factors leading to a specific patient-safety risk focusing on medication administration.
Explain evidence-based and best-practice solutions to improve patient safety focusing on medication administration and reducing costs.
Explain how nurses can help coordinate care to increase patient safety with medication administration and reduce costs.
Identify stakeholders with whom nurses would coordinate to drive safety enhancements with medication administration.
Communicate using writing that is clear, logical, and professional, with correct grammar and spelling, using current APA style.
Additional RequirementsLength of submission: 3–5 pages, plus title and reference pages.
Number of references: Cite a minimum of 4 sources of scholarly or professional evidence that support your findings and considerations. Resources should be no more than 5 years old.
APA formatting: References and citations are formatted according to current APA style. -

The journal is an essential assignment that is meant to sum up the conclusions you come to after having reflected on the readings in the text, the online lecture, discussion posts, including your own and those of your peers, and any outside material you consult. You will probably find that you do as much or more thinking than you do writing in responding to the journal prompts – and that is perfectly okay. In general, the weekly journal should meet the noted length requirement – not including the space needed for the prompts.
You may feel the need to write out longer reflections – and that is also okay. If you do feel the need for longer reflections, then, once you have written them out, try to edit them, reducing them to their essence.
Part of this course is process – training ourselves to think critically. Part of it is learning to understand how we think and why we think or believe what we think or believe. While the journal prompts will occasionally address the process, it will more often ask you to reflect on the how’s and why’s of what you know and believe – or what you think you know and believe. -
In this assignment, you are going to locate a conceptual framework or grand theo
In this assignment, you are going to locate a conceptual framework or grand theory that you could use in advanced nursing practice. After briefly explaining it, you will then describe how your chosen conceptual framework or grand theory of nursing relates to each of the metaparadigm concepts of person/client, nurse, health, and environment as described by the theorist. Lastly, you will apply the conceptual framework or grand theory to advanced nursing roles. In particular, how does this conceptual framework or grand nursing theory apply to advanced nursing practice, education, and research?

Hi, I need someone to respond to the following student discussion reply which I have provided below. Also, below the student discussion reply I have provided the original discussion prompt that the reply was referring to. Also, my state is Florida and my specialty is medsurge/Tele. I have provided an example of how to reply to a student to give you an idea of how to respond to a student discussion reply.
Student Reply That Needs to Be Responded to:
Hi,
Very interesting read. I was particularly interested in NPs and their practice across states this week while researching. I learned that they are also regulated by the state as far as the scope of practice into three categories: full practice, reduced practice, and restricted practice. Restricted laws “restrict the ability of NPs to engage in at least one element of NP practice. State law requires career-long supervision, delegation or team management by another health provider in order for the NP to provide patient care” (American Association of Nurse Practitioners, 2022). For reduced practice, state laws “reduce the ability of NPs to engage in at least one element of NP practice. State law requires a career-long regulated collaborative agreement with another health provider in order for the NP to provide patient care, or it limits the setting of one or more elements of NP practice (American Association of Nurse Practitioners, 2022). Full practice includes diagnosis, treating, ordering tests, prescribing medications, and managing treatments (American Association of Nurse Practitioners, 2022). My state, which is Wisconsin, is a reduced practice state which I never realized. Should I decide to move further into my nursing career, I’ll need to consider this as I presumed NPs had full practice in all states. Are you considering further nursing education as well? Were you aware of these restrictions for NPs by state? I noticed that Florida is a restricted practice state which is where you are from. What are your thoughts on that?
Original Discussion Prompt:
Outline the process for the development of nursing standards of practice for your state, including discussion of the entities involved in developing the standards of practice and how the standards of practice influence the nursing process for your areas of specialty.
Example of How to Respond to a Student Discussion Reply
Hi Nita,
I like how you discuss both the teacher observing the student and the student observing their own art. You bring up excellent points about the teachers observing the children, because it does help the teacher to better communicate with the children and understand them better. As teachers, it is very important to nurture students, particularly if they are very young, I think, because it allows them to better understand the needs of the student, and how to work with them. Paying attention to what the student is drawing, and seeing how the student reacts to their own work can improve the teachers’ understanding.
Expressing feelings in this discussion board reply examples is a major component of growing up. The more a student is able to express those feelings, the more they can understand them. I liked reading about what you said on this topic, and I agree that the art is a very useful tool that students can learn to express their feelings effectively. If they do not express them, they are keeping them inside, and drawing can become a form of counseling for them. In my opinion, I think the drawing can be particularly useful for many students who are quiet. These students do not often express themselves verbally, and drawing can be a way for them to understand their feelings, and to release some pent up sadness of hostile feelings, for example. I think this form of therapy sticks with many people throughout life, and they continue to use drawing as a coping mechanism.
Thanks,
Steven
