Scenario:J.T. is a 20 year-old who reports to you that he feels depressed and is experiencing a significant amount of stress about school, noting that he’ll “probably flunk out.” He spends much of his day in his dorm room playing video games and has a hard time identifying what, if anything, is enjoyable in a typical day. He rarely attends class and has avoided reaching out to his professors to try to salvage his grades this semester. J.T. has always been a self-described shy person and has had a very small and cohesive group of friends from elementary through high school. Notably, his level of stress significantly amplified when he began college.
You learn that when meeting new people, he has a hard time concentrating on the interaction because he is busy worrying about what they will think of him – he assumes they will find him “dumb,” “boring,” or a “loser.” When he loses his concentration, he stutters, is at a loss for words, and starts to sweat, which only serves to make him feel more uneasy. After the interaction, he replays the conversation over and over again, focusing on the “stupid” things he said. Similarly, he has a long-standing history of being uncomfortable with authority figures and has had a hard time raising his hand in class and approaching teachers. Since starting college, he has been isolating more, turning down invitations from his roommate to go eat or hang out, ignoring his cell phone when it rings, and habitually skipping class. His concerns about how others view him are what drive him to engage in these avoidance behaviors.

Advanced Practice Nurses (APNs) and Nurse Practitioners (NPs) are professional nurses with a gigantic knowledge base and
clinical practice skills. These two groups of registered nurses usually utilize clinical competencies to ensure effective practice and
delivery of holistic care based on the guidelines within their scope of practice (Schober et al., 2020). The dynamic changes in the U.S
healthcare system have greatly diversified the professional roles of APNs and NPs in clinical settings. This discussion will explore various
differences and similarities in APNs and NPs’ roles, and how those roles can be communicated to clients and other healthcare
practitioners.
Role Similarities
To start with the similarities, both APNs and NPs offer holistic nursing services in diverse healthcare settings, including clinics,
long-term care facilities, hospitals, and outpatient facilities. The similar professional roles of APNs and NPs include conducting nursing
diagnosis and patient assessments (Lowe et al., 2012). Additionally, both NPs and APNs prescribe medications depending on the state
laws or regulations. Depending on state regulations on the scope of practice, both NPs and APNs also admit patients and are actively
involved in discharge planning (Lowe et al., 2012). Another similar role between the two groups of MSN-prepared nurses is providing
patient-centered health education to specific patients and their families.
Role Differences
Despite the identified similarities, APNs and NPs have some different professional roles. First, APNs specializations includes roles
like midwifery, clinical nurse specialists, or nurse anesthetist (Schober et al., 2020). On the other hand, NPs generally perform their
professional roles or duties as primary care providers, specifically in clinics or private practice because they can practice independently
depending on state regulations. The roles of NPs are expanded in states where MSN-prepared registered nurses have the prescriptive
authority and can practice independently from physicians. Second, unlike NPs, the APNs require certification to practice in specialty
settings. Generally, the roles of NPs and APNs vary considerably because NPs’ roles majorly focus on primary care while APNs provide
specialized care (Schober et al., 2020).
Communicating the Roles
When communicating to other healthcare providers and potential consumers concerning the APNs and NPs’ roles, it is important
to specify the areas of professional practice for the two nursing groups (Schober et al., 2020). It is essential to ensure that clients and
other healthcare practitioners understand that Advanced Practice Nurses specialize in various areas, including geriatric and women’s
health (midwives). Moreover, it is crucial to inform clients and other healthcare providers that Nurse Practitioners provide a wide range
of professional health care services, particularly in primary care setting (Lowe et al., 2012). Generally, highlighting the scope of practice
for APNs and NPs would allow clients and other healthcare practitioners understand how their professional roles may slightly differ
depending on the state regulation or laws.
Conclusion
In conclusion, the professional roles of APNs and NPs are important in different healthcare setting because the two groups of
MSN-prepared nurses help meet the holistic needs of individual patient by emphasizing on comprehensive health assessment, health

Prescriptive Authority
One barrier that has slowed down the progress of advanced practice nursing is the prescriptive authority based on each state. There are three divisions that the
American Association of Nurse Practitioners have created to categorize each state. These include restricted, reduced, and complete or full. If a state allows full practice
authority, the advanced practice nurse may autonomously prescribe medications equivalent to a physician. There are twenty-two states that allow full practice authority.
In states that have reduced and restricted prescriptive authority, the advanced practice nurse requires supervision by a physician (Zhang & Patel 2022).
There are sixteen states who allow reduced practice authority. The last twelve states are fully restricted. The states that have reduced prescriptive authority have
different limitations for the medications that advanced practice nurses may prescribe. In the states that are restricted, the advanced practice nurse needs direct
supervision from a physician when prescribing controlled substances (Zhang & Patel 2022).
Over time, more strategies to overcome prescriptive authority are emerging. In the 1990’s, progress was made as the Controlled Substance Act allowed advanced
practice nurses to have prescriptive authority, but they also required authority by the DEA. The advanced practice nurses needed a DEA number tied to the physician
they were working with, which was found to be restricting. Later that year, they amended the Controlled Substance Act, and mid-level practitioners were able to obtain
their own DEA numbers in states where there was full prescriptive authority (Hamric et al., 2013). There is still progress that needs to be made but over time the
advanced practicing nurse’s ability to prescribe is expanding.
Identified Practice Environments
Another barrier that has slowed down the progress of advanced practice nursing is each state’s identified practice environments described as full, restricted, and
reduced. This is a barrier that interferes with an advanced practice nurse to provide care with their full educational training. When a state allows full practice authority,
the advanced practice nurse may evaluate, diagnose, interpret, and order diagnostic tests, initiate treatments, and prescribe controlled substances under their state’s board
of nursing license. In both reduced and restricted states, advanced practice nurses are not able to practice at least one domain that a full practice state allows. In some
states, a physician is required to delegate and continuously supervise advanced practice nurses for them to practice. This interferes with advanced practice nurses and
their full ability to provide care (Kleinpell et al., 2022).
Strategies that may help overcome these barriers include involving advanced practice nurses in the credentialing process within each organization and creating a
practice committee to support advanced practicing nurses. Another strategy is making sure we stay within our scope of practice when prescribing and assessing. These
strategies may assist in allowing advanced practice nurses to practice within their full extent of training and education. This leads to better patient care and outcomes
with a higher quality of care (Kleinpell et al., 2022).
Payor Policies
One last barrier that has slowed down the progress of advanced practice nursing is payor policies. Advanced practice nurses are reporting that payor policies have
a large, negative impact on practicing to the full extent of their education and training. Since advanced practice nurses may have a more restrictive scope of practice in
some states, this leads to payor policies being stricter and preventing advanced practice nurses from practicing independently. Many times, commercial health plans do
not recognize advanced practice nurses as primary care providers and must bill under a physician’s name, which leads to not paying the nurse practitioner directly or
reimbursing them at a lower rate (Hain & Fleck, 2014). A possible strategy for overcoming this barrier is to remove the barriers that are preventing advanced practice
nurses to practice independently. Advanced practice nurses have contributed to high quality primary care; therefore, they should be provided equitable pay (Hain &
Fleck, 2014).
