    Suturing is a technique that has been around for thousands of years to close cutaneous wounds (Ratner, 2020). The primary goals of suturing have remained the same: closing dead space, supporting and strengthening wounds until healing increases tensile strength, approximating skin edges for an aesthetically pleasing and functional result, and minimizing the risks of bleeding and infection (Ratner, 2020). There are multiple suturing techniques, and the choice depends on the type and location of the wound, thickness of the skin, degree of tension, and desired cosmetic result (Ratner, 2020). Suture techniques include simple interrupted, simple buried, vertical mattress, horizontal mattress, figure-of-8, half-buried, simple running, simple running locking, and subcuticular running (Zenn, 2013). Interrupted sutures are the most common and easy to place, have high tensile strength, and decrease the risk of edema and impaired circulation (Ratner, 2020; Zenn, 2013). However, interrupted sutures take longer to place and can often cause crosshatched marks or scarring across the suture line (Ratner, 2020). Simple buried sutures are the main suture that will be used when deep sutures are needed before skin sutures; for these sutures, it is important to start deep and end superficially to avoid buttonholing the skin (Zenn, 2013). Simple running sutures are more appropriate for longer wounds with deeper placed sutures that have already minimized wound tension whereas running locked sutures have increased tensile strength (Ratner, 2020). Vertical and horizontal mattress sutures maximize wound eversion, reduce dead space, and minimize tension across the round (Ratner, 2020). Half-buried vertical and horizontal mattress sutures are reserved for cosmetically important areas such as the face and provide better cosmetic outcomes (Ratner, 2020). Running subcuticular sutures can be used when space is minimal and dead space has been eliminated; the epidermis is only penetrated at the beginning and end of the suture line, minimizing crosshatching and provided the best possible cosmetic outcome (Ratner, 2020). In general, the success of any suturing technique has a lot to do with appropriate selection of sutures, needles, and needle holders (Ratner, 2020). The technique that is selected should allow for the smallest suture that will adequately hold the wound edges while ensuring the tensile strength of the suture does not exceed the tensile strength of the tissue (Ratner, 2020).

    Suture is considered a standard of care in primary wound closure. It is helpful in minimizing bleeding and aides wound in the healing process (Azmat & Council, 2020). The purpose of sutures is to reduce the dead space between the wound opening and provide the necessary support needed for the skin heal appropriately. Before the suturing process, the provider must evaluate the skin and wound type, the degree of tension, as well as the anatomical location. It is also important to ask for patient’s preferences and what their desire cosmetic skin results (Azmat & Council, 2020). It is essential to acknowledge infection risk and inflammatory reaction associated with the suture material prior to using the appropriate suture for wound closure (Azmat & Council, 2020).
    There are two different types of sutures; absorbable and non-absorbable. Absorbable sutures are intended to stay inside the body after closure and will absorb overtime, they do not require a provider to remove them, whereas, non-absorbable sutures do not degrade and will need to be removed later (Gonzalez, 2018).
    There are various types of suture selections techniques that can be used when suturing. Continuous suturing involves a single strand of suture material but include a series of stitches. This can be done fast compared to other sutures and is evenly distributed throughout the entire wound. They can be used on the dermis or in internal organs such as the uterus. They are great for achieving hemostasia. Interrupted sutures are the most common in skin wound and are done one closure at a time. Single, vertical, horizontal, and pulley are different approaches to interrupted sutures. Deep sutures are a type of suture that is placed under the layers of tissue below the skin. Buried sutures are done by using absorbable sutures and not able to be seen from the outside as they are buried under the skin. It can be done single or continues. Purse-string sutures are a continuous suture placed around an area and tightened like the drawstring. Lastly, subcutaneous sutures are placed in the dermis and subcutaneous tissue, which lies below the upper layer of the skin (Gonzalez, 2018).

    Psychiatric Mental Health Nurse Practitioners (PMHNPs) can provide comprehensive primary care services like psychiatrists (Chapman et al., 2018). They can conduct assessments and manage disorders through evaluation, psychotherapy, and medication management. Despite the increasing demand for PMHNPs, some states in the U.S. have limitations on their ability to practice to the full extent of their education and scope. Research has demonstrated that the quality of care PMHNPs provide is comparable to that of psychiatrists, resulting in positive health outcomes for the patients they provide services to; however, they are not reimbursed at physicians’ Medicaid fee-for-service rate.
    The Health Resources and Services Administrations Bureau of Health Workforce has identified more than 4,700 areas with a shortage of Mental Health Professionals. This shortage affects over 100 million people in total. Around 62% of these areas are in partially rural regions. Among health professionals’ psychiatrists are particularly scarce, and their numbers are decreasing. According to this analysis, the ratio of psychiatrists per 100,000 U.S. residents decreased by 10.2% between 2003 and 2013. This decline can be attributed to physicians not applying for psychiatric residency training and many psychiatrists approaching retirement age. Moreover, PMHNPs have the potential to address workforce shortages in healthcare in underserved areas. Their impact on health services is particularly significant for women, racial minorities, rural populations, and individuals struggling with substance use disorders (Chapman et al., 2018).
    State regulations play a role in determining the availability of PMHNP services (Chapman et al., 2018). When states grant practice authority to PMHNPs, it leads to an expanded nurse practitioner (NP) workforce and improved healthcare access, particularly in rural areas. Some stakeholders recognize the contributions of PMHNPs to the health workforce and advocate for support in terms of training and payment while also calling for removing barriers imposed by federal and state laws. However, limited research exists regarding the implications of PMHNP contributions within practice settings (Chapman et al., 2018).
    Another vital issue is how PMHNPs are valued and paid. While there are models of the PMHNP’s impact as direct care providers, they do not fully cover all the factors related to the types of services billed by PMHNPs; these projections also do not include the costs involved in physician supervision in states where it is necessary (Barnes et al., 2016). One study illustrated the effect reimbursement rates have on NPs. A national sample was taken of 252,657 ambulatory practices to examine the outcomes that state policies had on NP employment in primary care and practice Medicaid acceptance; the odds were 13% higher in states that allowed NPs to practice to the full extent of their scope, interestingly, those odds increased to 20% in states that gave NPs 100% reimbursement at the physicians Medicaid fee-for-service rate. Moreover, the odds of NPs accepting Medicaid compared to those who do not were 23% higher (Barnes et al., 2016).
    The growing need for healthcare services in mental health has emphasized the potential contribution of PMHNPs as a solution. To enhance NP involvement in care and improve access to healthcare, lifting restrictions on the scope of practice and enhancing Medicaid reimbursement rates are required (Barnes et al., 2016). These strategies will ultimately address provider shortages while improving patient care. Achieving this goal requires NPs to play an active role in the advocacy of health policy reform.

    A good literature review should involve first knowing what topic or general idea to search. Based on this idea, the search can begin on academic databases using keywords or similar words to that topic to find related research. The purpose of a literature review is to find already existing articles on this topic and to synthesize those findings to discuss and connect this research (Tappen, 2016). An important part of any literature review is to identify the source of the material and ensure that it is academically sound based on where it was published, who the authors are, and the methodology of the study (Tappen, 2016). To ensure that I write a successful literature review, I will first research my topic and the surrounding history of it. This should be followed with more current trends especially related to nursing turnover and retention to create a picture of the reasons why nurses leave and some possible solutions that these studies have found to retain nurses within an institution. Some challenges that may come up are implicit bias from the authors especially if they have a possible conflict of interest in writing the journal. It is always important to vet sources to determine their credibility and applicability to the topic in question.

    What are some of the most critical components of a good literature review?
    An important concept of conducting a good literature review is performing a thorough search of present literature in order to determine what is known and not known about the chosen topic. Oftentimes, this can be the most challenging and time-consuming part. According to Tappen (2015), there are many helpful tricks that can make the process easier and promote the best outcomes, including but not limited to:
    1) using a variety of search terms that relate to the topic
    2) using a variety of resources, including scholarly articles within search engines, online databases, reports, books, etc.
    3) reviewing the reliability and credibility of the source, especially if it is an unfamiliar one
    4) reviewing older resources, even ones that are 15-20 years old, to find the original, theoretical work on the topic 5) ensuring the resources are unbiased
    How will you ensure that you write a successful literature review for your planned research proposal?
    To ensure that I write a successful literature review, I would follow those aforementioned steps in order to conduct a thorough literature search. I would first search for different resources and compile them all for review. This allows me to go in with an open mind and look at all of the resources as a whole before I start choosing which resources to include in my literature review that will address all the components I need. I would organize the different resources in a way that would suit an outline for my literature review and then thoroughly, critically read them and take notes. After analyzing the material and making conclusions, ensuring every component is addressed, I would then write my literature review.
    What are some of the challenges you foresee when writing your literature review?
    Some challenges I anticipate in writing my literature review include allocating sufficient time to conduct a thorough search of literature, thoughtfully organizing the literature in my rough publish in a way that makes sense and finding the exact research I need for my specific population I chose (adult females). Time efficiency is crucial with assignments such as this because there are so many resources available that it takes time and effort to go through them all to identify what is relevant, reliable, credible, etc. Because my specific population was centered on adult females, it narrows the search even more so I am hopeful that this does not pose too much of a challenge in finding relevant research. It is evident how important choosing key words are in conducting research.
