CASE STUDY BELOW:
Juan Duran is a Mexican American.
He is at Diabetes Clinic at the VA.
He is exceedingly polite and respectful of health care personnel (and does not like to interrupt or ask too many questions).
He has been in the United States more than 50 years; he is a Navy veteran.
Juan is wandering around the hallway and is found by Mr. Carlson, the clinical director.
Mr. Duran tells him that he has been instructed to start insulin for his diabetes but doesn’t know how. He doesn’t recall receiving any appointment or instructions.
Mr. Carlson finds the Diabetes Nurse Educator, Jenny O’Connell, and asks her to fit Mr. Duran in for an unscheduled appointment.
Nurse Jenny finds out more about Mr. Duran. He:

IntroductionNurses have a powerful role in the coordination and continuum of care. All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses. Understanding policy elements helps nurses coordinate care effectively.
This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. You are encouraged to complete the Managing Change activity.

PreparationYour nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics. Select a client from clinical experience with an acute health problem or complaint requiring at least two visits. Submit a complete H & P SOAP note from the initial visit with this client, as well as a focused SOAP note for the follow-up visit. Based on this client’s condition, conduct a literature search for two research articles that discuss various approaches to the treatment of this condition. Peer-reviewed articles must address the standardized procedure or guidelines for this diagnosis. Incorporate the research findings into the decision-making for this client’s treatment. In the paper, compare and contrast or address how treatment or the plan may have been different based on the research findings. The theoretical framework is a critical element of your work. A theoretical framework is the general representation of relationships in your problem and is based on existing theory.
The conceptual framework is your idea on how the problem should be explored, and it is generally based on a theory. The difference between the two is the scope. Theory is broad; concept is narrow.
In most research studies, your framework can be either theoretical or conceptual in nature. For the purposes of this project, you will most likely follow a conceptual framework, which is your idea on how the research problem will be explored (although you could have both).
For example:

Conceptual framework: A new teaching method is applied to elicit improvement in midterm test scores.
