    He is at Diabetes Clinic at the VA.

    Juan is wandering around the hallway and is found by Mr. Carlson, the clinical director.

    Mr. Carlson finds the Diabetes Nurse Educator, Jenny O’Connell, and asks her to fit Mr. Duran in for an unscheduled appointment.

All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses.     The conceptual framework is your idea on how the problem should be explored, and it is generally based on a theory. The difference between the two is the scope. Theory is broad; concept is narrow.
    In most research studies, your framework can be either theoretical or conceptual in nature. For the purposes of this project, you will most likely follow a conceptual framework, which is your idea on how the research problem will be explored (although you could have both).
    For example:

    Conceptual framework: A new teaching method is applied to elicit improvement in midterm test scores.
