Pulmonary Sarcoidosis
A 40 year old white male pt, N.N., to ER with c/o fatigue, swollen lymph nodes, persistent cough without phlegm, dyspnea. Pt is 6 feet tall, 190lbs. After extensive study, via 6MWT, CT imaging, PFT, pt was told to have pulmonary and cardiac sarcoidosis.
VS: 98.7F, 88, 20, 146/90, 92%RA dropping to upper 80s with exertion; no pain
Meds: prednisone 10mg daily, methotrexate 7.5mg/d
Full code with older sister, Esther, as the durable power of attorney for healthcare. No one in family has been affected by the disease.

How healthy is your workplace?
You may think your current organization operates seamlessly, or you may feel it has many issues. You may experience or even observe things that give you pause. How Public Health Nurses Make the Case
Public health nurses working for a local or county health department often find themselves more involved with program planning than direct patient care. Often, they are asked to help develop a health education program based on what is affecting the community the most, limited by budgetary constraints while still meeting reporting requirements of grant funders.
Let’s say your health department has been given the opportunity to apply for funding from three sponsoring organizations. The choices are to develop public health education programs about alcohol consumption, smoking cessation, and community HIV testing. In an upcoming department meeting you are asked to make a case for and against applying for the funding of each of these programs.

2.) Deconstruct each rate-based measure to include the following:
a. Describe the definition of the measure.
b. Explain the numerical description of how the measure is constructed (the numerator/denominator measure counts, the formula used to construct the rate, etc.).
c. Explain how the data for this measure are collected.
d. Describe how the measurement is compared externally to other like settings, and differentiate between the actual rate and a percentile ranking. Be specific.
e. Explain whether the measure is risk adjusted or not. If so, explain briefly how this is accomplished.
f. Describe how goals might be set for each measure in an aggressive organization, which is seeking to excel in the marketplace. Be specific and provide examples.
4.) Describe the importance of each rate-based measure to a chosen clinical organization and setting.
a. Using the websites and resources, you can choose a hospital, a nursing home, a home health agency, a dialysis center, a health plan, an outpatient clinic, or private office; a total population of patient types is also acceptable, but please be specific as to the setting. That is, if you are interested in patients with chronic illness across the continuum of care, you might home in a particular health plan, a multispecialty practice setting or a healthcare organization with both inpatient and outpatient/clinic settings.
b. Note: Faculty appointments and academic settings are not permitted for this exercise. For all other settings, consult the Instructor for guidance. You do not need actual data from a given organization to complete this Assignment.

Discussion Reply
One of my experiences with a provider was negative. I went to a new primary care provider. He
noticed that my systolic blood pressure was slightly elevated in the 140s, and my BMI was 29. I
told him that my blood pressure is usually in the normal range, but it is high today because I had
one of the most stressful times at work as a nurse and in school to be a family nurse practitioner.
The PCP did not listen and stated that I needed to see a cardiologist and exercise more. I told the
PCP that I am a runner and an avid bodybuilder. The PCP could see I was in shape, and even
after I told him I exercise often, he reiterated that I needed to exercise. I told the PCP that I could
lower my BP by changing my diet and decreasing my stress, especially being only slightly high.
Lifestyle modifications, such as reducing salt to 1.5 gm per day and decreasing saturated fat,
alcohol, and caffeine intake, should be initiated first to decrease mild hypertension (Aronow,
2017). The PCP did not want to listen to my options, although he knew I was an RN and I was in
my last semesters of FNP school. In the shared decision model, there are five steps to making
decisions of care (Agency for Healthcare Research and Quality [AHRQ], 2020): seeking the
patient’s participation; exploring options with the patient; assessing the patient’s preferences;
reaching a decision with the patient; evaluating the decision. The PCP did not do any of the steps
in the shared decision-making model. The PCP made me angry enough to want to search for a
new PCP, but luckily, he changed his position to a hospitalist; therefore, I had to search for a
new PCP.
