Title: What makes a team effective or ineffective in terms of achieving expected outcomes for the patients?
In patient care, a variety of people in different roles making up a team collaborate to assist patients with their healthcare. This multidisciplinary team can include nurses, advanced practice nurses, administrative staff, other providers, and more. Rosen et al. (2018) explained that in order to deliver safe and high-quality care, there must be reliable teamwork, as well as collaboration. When providing healthcare, there is collaboration between the patient and families and the health providers. Those of the health care team share responsibility and accountability when it comes to the care provided. If an error were to occur, it is important for others in the healthcare team to have accountability for the error and to learn from it to prevent another occurrence in the future. Buljac-Samardzic et al. (2020) noted that having a safety culture where those of the healthcare team are able to debrief on a difficult situation and an error that occurred, and being able to learn from this situation, is an important team aspect when it comes to providing safe patient care. This debriefing requires those of the multidisciplinary team to be comfortable with recognizing and reporting these situations (Rosen et al., 2018). In relation to this, it would also be important for those of the healthcare team to have self-awareness and be comfortable reaching out to others in the team when unsure about the proper path of treatment to take, for example.
Whether it is debriefing on a difficult situation or discussing possible treatment options, all healthcare provided to patients involves communication among the team. Poor communication can cause a team to be deemed ineffective in terms of achieving expected outcomes for patients. In healthcare, communication is always occurring. Whether it is during transition of care in the acute setting or in relation to medication administration, for example, communication is important to prevent patient harm (Rosen et al., 2018). If a pharmacist and provider do not properly communicate the medication, dosage, and administration route properly, it could result in a medication error that is preventable. In addition to this, when giving handoff report on a patient in an acute setting, it would be important to properly communicate critical information regarding the patient’s status and plan of care so that delays in treatment or inappropriate treatments can be avoided (Rosen et al., 2018).
With the team being made up of a variety of people with many different roles in patient care, there are many different views and different levels of experience present. With having a diverse group of people providing care to a patient, diverse input of knowledge can be present (Rosen et al., 2018). When discussing patient care and the route to be taken with their care in order to achieve the best outcome, having a group of people with different perspectives may be beneficial. By discussing various ideas regarding promotion of health and prevention and treatment for a patient, a consensus could be reached in relation to which option could provide the best outcome for the patient. Overall, the healthcare team involves those that are directly involved with patient care and directly influence the outcome of the care the patient receives. With proper communication, accountability, responsibility and collaboration, the team can be effective in providing safe and effective care to patients.
