Case Study: You are called to a postpartum room and find Michael, a two-hour-old infant, irritable and spitting up copious amounts of formula. You do not know the maternal history, so you advise the mother that you would like to return the baby to the nursery for assessment and monitoring. In the medical record, you focus on the prenatal history and find the mother had an addiction to opioids with her last pregnancy but denied drug use with this pregnancy. You witness the baby having some significant tremors, vital signs: T (axillary) – 36.0, RR- 78, HR- 166, and the infant is alert, irritable, and does not console easily. The infant is constantly moving and sucking vigorously on the pacifier.
What do you think is occurring with the infant?
What lab test can be ordered to confirm your suspected diagnosis?
What interventions can be implemented to treat this infant?
At least 120 words and address all questions. Thank you.

  • As an advanced practice nurse assisting physicians in the diagnosis and treatmen

As an advanced practice nurse assisting physicians in the diagnosis and treatment of disorders, it is important to not only understand the impact of disorders on the body, but also the impact of drug treatments on the body. The relationships between drugs and the body can be described by pharmacokinetics and pharmacodynamics.
Pharmacokinetics describes what the body does to the drug through absorption, distribution, metabolism, and excretion, whereas pharmacodynamics describes what the drug does to the body.
When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (i.e., diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.
For this Discussion, you reflect on a case from your past clinical experiences and consider how a patient’s pharmacokinetic and pharmacodynamic processes may alter his or her response to a drug.
    RESOURCES
Be sure to review the Learning Resources before completing this activity. Click the weekly resources link to access the resources. WEEKLY RESOURCES
    To prepare:
Review the Resources for this module and consider the principles of pharmacokinetics and pharmacodynamics.
Reflect on your experiences, observations, and/or clinical practices from the last 5 years and think about how pharmacokinetic and pharmacodynamic factors altered his or her anticipated response to a drug.
Consider factors that might have influenced the patient’s pharmacokinetic and pharmacodynamic processes, such as genetics (including pharmacogenetics), gender, ethnicity, age, behavior, and/or possible pathophysiological changes due to disease.
Think about a personalized plan of care based on these influencing factors and patient history in your case study.
BY DAY 3 OF WEEK 1
Post a description of the patient case from your experiences, observations, and/or clinical practice from the last 5 years. Then, describe factors that might have influenced pharmacokinetic and pharmacodynamic processes of the patient you identified. Finally, explain details of the personalized plan of care that you would develop based on influencing factors and patient history in your case. Be specific and provide examples.
BY DAY 6 OF WEEK 1
Read a selection of your colleagues’ responses and respond to at least two of your colleagues on two different days by suggesting additional patient factors that might have interfered with the pharmacokinetic and pharmacodynamic processes of the patients they described. In addition, suggest how the personalized plan of care might change if the age of the patient were different and/or if the patient had a comorbid condition, such as renal failure, heart failure, or liver failure.

    A. Compare public healthcare policies at the federal and state level that affect the provision of patient care and healthcare delivery by doing the following:
    1. Describe one federal and one state public healthcare policy that affect the provision of patient care and healthcare delivery.
    2. Explain how each policy works (e.g., Is it mandatory? Will enforcement be necessary? Who is responsible for administering the policy?).
    3. Discuss each policy’s impact on the population(s) by answering the following questions:
    • What populations will benefit?
    • How will they benefit?
    • What populations will be negatively impacted?
    • How will they be impacted?
    • Does the policy impact health disparities and health equity?
    4. Discuss the evidence that informs each policy, including two scholarly evidence sources published within the last five years to support your discussion.
    a. Discuss whether the evidence identified in part A4 supports the chosen policies, including any modifications that should be made to each policy based on the identified evidence.
    B. Explain the role of the nurse in policy development and how the nurse could impact the administration of, or the revision of, a policy at both federal and state levels.
    1. Reflect on barriers to the nurse’s engagement in each of the following healthcare policy areas:
    • development
    • administration
    • revision

    Hidden Gem
The Hidden Gem is a pathway that takes small parts, or “gems,” from each model. It’s a good fit if there are truly wonderful parts of both models that you don’t want to let go of, but in large part the models don’t work. Refer to Figure 7-1, Starting Point for a Hidden Gem on page 149 of Creating Great Choices.
Example: Imagine you’re a television producer of The Bachelor or The Bachelorette, popular American television series in which a single man or woman auditions potential romantic partners in hopes of finding a husband or wife. Each week, the Bachelor or Bachelorette hands roses to the contestants they wish to continue seeing. If a contestant doesn’t get a rose, it means that they’re leaving the show. To make a show like this work, it’s important that producers find contestants with good personalities that audiences like to watch.
The Problem: As the producer, you notice that some of the contestants that audiences love to watch aren’t being given roses. For instance, Evan, from The Bachelorette season 12, was goofy, cringeworthy, but lots of fun. Likewise, Carly, from The Bachelor season 19, was so fun and bubbly, and you wanted the Bachelor to keep her around just to get to hang out with her. However, neither Evan or Carly were chosen, and both had to leave the show.
As the producer, you need to find a way to capitalize on the popularity of Evan and Carly. What do you do? The Hidden Gem solution ended up being a new show called Bachelor in Paradise, which only featured previous contestants. In other words, the producer took the small working parts from the other TV show and created a new one.
