The project will involve the use of cognitive behavioral therapy in individuals
The project will involve the use of cognitive behavioral therapy in individuals diagnosed with depression and the PHQ-9 questionnaire will be used to assess for an absence or decrease in severity of symptoms after receiving the cognitive behavioral therapy. This will take place over 8 weeks and the PHQ-9 questionnaire will be administered prior to beginning therapy and at the end of therapy to compare the scores and evaluate the effect of the the cognitive behavioral therapy. This will be completed at an outpatient family practice clinic.
The identification of a significant practice problem and associated project need are the cornerstone of a practice change project. The purpose of this assignment is to conduct an organizational needs assessment. The formulation of a comprehensive organizational needs assessment supports the professional formation of the DNP-prepared nurse. To complete the assessment of the organizational need, you will need to interview a key decision-maker at the practicum site. If you are not implementing your DNP Project at a practicum site, complete the assignment as if you had interviewed a key decision-maker at a practicum site.

Practice Gap Identification
Directions: After performing a needs assessment with the practicum site decision maker(s), use the following table to help you understand what gaps are occurring between current practice and evidence-based practice (EBP). An example is provided for you below.
Example: Pediatric Practice
What is currently happening at the practicum site? What should be happening at the practicum site based on current evidence? What is your practice gap? (state what is currently happening and state what should be happening) Why is there a practice gap? What factors are contributing to the practice gap? What evidence do you have to demonstrate there is a practice gap?
There are no clinical practice guidelines for the management of obese patients in a pediatric setting being used at the practicum site. Nurse Practitioners (NPs) should be using evidence-based clinical practice guidelines to better manage children who are obese Approximately 40% of the patient population have been identified as being obese; however, there are no clear processes/guidelines in place for the NPs to use/follow when planning patient care.
Potential issue with lack of knowledge associated with updated guidelines for pediatric obesity.
Physician-owned facility; potential lack of NP autonomy regarding practice. Comments provided by the NPs.
NPs report they want to be able to better manage and plan care for pediatric patients who are obese.
Pediatric obesity prevalence is increasing in the clinic practice year over year.
Practice Gap Analysis Table: (This is the actual questions that need to be answered and placed in the paper the paper to complete the table)

Discussion 1 U2dis1- (word document attached)———–Using the Riverbend City
Discussion 1 U2dis1- (word document attached)———–Using the Riverbend City: Strategic Planning for Community Needs media piece you completed in this unit’s study, focus on the public health issues and challenges each character described. The community is clearly at a critical inflection point where strategic approaches should be enacted to address the community’s pressing public health challenges. But where and how should the director of the local public health department begin?
Drawing from key learnings in your readings, identify and describe three of the six approaches to planning you feel the director of the local public health department should consider. Specifically, reference the community members’ concerns in your response. For each approach, provide a definition, the condition(s) for which it is best applied, and why you selected the approach.
Discussion 2—-(document attached )
Goals and Outcomes in Context
In the Riverbend City: Community Health Needs Assessment media piece you completed as part of this unit’s study, you had the opportunity to prioritize the public health issues voiced by the citizens. Now, apply what you have learned to a real-life community based on the Hennepin County Medical Center Community Health Needs Assessment, linked in the Resources.
Utilize the Initial List of Health Needs for Consideration section of the Hennepin County Medical Center Community Health Needs Assessment (pages 14–21) to complete the following: Review the 7 listed health needs, and select one. Be sure to identify your selection.
Relate your selection to 2–3 of the key qualitative themes from interviews for that health need. Use the data provided for the county, HCMC community, and specific neighborhood/population to further describe the health need you selected.
Align the health need and the key qualitative themes you have selected to the most significant health outcome you would expect to achieve at a community level. Be sure to explain your rationale.
DISCUSSION 3—-Chapter 5 of your textbook discussed the public health pyramid (Figure 1-2). At each level of this pyramid, health problems, relevant theories, and targeted interventions are delivered.
For this discussion post, do the following:
Refer to a specific level of the public health pyramid. This should be clearly named in your post.
Utilizing the health need you identified in this unit’s Goals and Outcomes in Context discussion, conduct a brief literature search of evidence-based interventions regarding the chosen health need. Identify and briefly describe the intervention. Remember, alignment is key here—if you identify “direct healthcare services” in the public health pyramid, the evidence-based intervention should in fact be one-to-one (at the individual level).
Cite and explain how the chosen evidence-based intervention has been utilized to impact a specific health outcome. (This does not have to be the outcome you identified in the Goals and Outcomes in Context discussion.) -

Course Competency :
Design plans for care specific to the older adult.
Consider the scenario below, then follow the instructions underneath it to complete the assignment.
Mrs. Y
Mrs. Y is an 84-year-old client who was recently discharged from the hospital for an infected diabetic ulcer on her left leg. During her hospitalization, Mrs. Y required intravenous antibiotic therapy through a peripherally inserted central catheter (PICC) line.
Due to Mrs. Y’s long history of diabetes, her physician ordered that intravenous antibiotic therapy be continued at home. Subsequently, home health services were initiated, a home health nurse was assigned to Mrs. Y’s case, and an initial home visit was scheduled.
The home health nurse arrives at Mrs. Y’s home and introduces herself to the client and the family. The nurse explains the home nursing services that will be provided, including the PICC line and intravenous antibiotic therapy treatments.
During the initial home visit, the nurse assessed the physiological, psychological, functional, and safety needs of the client. The nurse’s findings were as follows:
Mrs. Y lives alone; however, her daughter checks on her frequently throughout the day.
The client is noted to have moderate functional issues and ambulates with a cane.
The client has several throw rugs in the main walking quarters and minimal lighting throughout the hallways.
Mrs. Y states “I used to get around my house with ease, but now I get a little tired and have to sit down and rest frequently.”
Consider Mrs. Y’s current health status and functional decline, then address the following:
Download the Concept Map and Plan of Care worksheet below. An example is also provided for your reference.
Concept Map and Plan of Care worksheet

Identify three (3) priority nursing diagnoses for Mrs. Y. Visit the School of Nursing Guide Nursing Reference eBooks section for resources to assist with nursing diagnoses.
Create a visual representation of the three (3) priority nursing diagnoses by incorporating them into the Concept Map (template in the worksheet). Be sure each nursing diagnosis includes the following elements:
“related to (r/t)” — description of the client’s problem
“as evidenced by” — description of the client’s symptoms
Complete the Nursing Plan of Care (table in the worksheet) describing what should be implemented for Mrs. Y.
Goals: Establish at least one (1) goal for each of the nursing diagnoses you identified (for a total of 3 goals). Goals should be: patient specific, measurable, actionable, realistic, and time limited.
Nursing Interventions: Describe at least three (3) nursing interventions for each of the goals (for a total of 9 nursing interventions). Each intervention should be in alignment with the goal it is supporting.
Complete the assignment using proper spelling, grammar, and APA.

Part 1: Evaluation and Evidence-Based Practice in Recreational Therapy
Case:
Margaret Mae is a 70-year-old retired therapeutic recreation university professor. She has no siblings or family members but maintains a close relationship with her best friend and colleague, Cheryl. She has two dogs that she loves deeply and has kept horses and an array of pets for years. Cheryl is her primary caregiver and support. She lived on a ranch in a rural community and was recently relocated to the assisted living facility. She enjoyed walks, visits to old friends, church, and concerts while able to travel. Margaret was highly respected in the field of recreation and very social. Cheryl first noticed Margaret showing signs of memory lapses; for example, losing reader glasses, poor driving judgment and position on the road, difficulty with checkbook calculations, forgetfulness, changes in reasoning capabilities, unnecessary purchases including a second home approximately two years prior to diagnosis. Cheryl voiced concerns to her primary care physician, but no testing was done until much later. Diagnostic MRI confirmed Cheryl’s suspicion of senior vascular dementia and Alzheimer’s dementia. Margaret became unable to care for herself and was placed in an assisted living facility. She was moved to your facility after an incident of elopement. You are a CTRS at the assisted living facility where Margaret now resides. You review
Significant past medical history:
– Family history of Alzheimer’s dementia, father and maternal grandmother.
– Mild hypothyroid treated with medication.
-Rheumatoid arthritis with minimal impact to mobility.
– Client had an incident of being overmedicated with psychotropic drugs, which led to a period of severe reduction in her overall function and ability to communicate and ambulate and masked a stroke that occurred; stroke induced aphasia.
– Once taken off these drugs, she began a 6-month period of regaining better overall functioning but never fully recovering
Medications: Levothyroxine (Synthroid) once daily.
Cognition/perception:
-Alzheimer’s dementia. Client has been admitted nonverbal but is alert and aware of environment.
– She reacts with head nods and smiles.
-She communicates with body language and occasionally will speak a few simple words.
Aphasia is present.
